Overview
Pretty much every single weapon type has a unique version. For the most part, they simply are more powerful and have better chances to crit, but they can also increase the clip size, add special damaging effects, increase the amount of crit damage, or even fire more than one mini-nuke at a time! There's also a handful of unique sets of clothing and hats that usually add nice extra benefits.
Small Arms
The Terrible Shotgun (Combat Shotgun)
The Terrible Shotgun is the personal weapon of Smiling Jack at Evergreen Hills Bazaar. You can obtain it by killing him, or by shooting it out his hands.
Ol' Painless (Hunting Rifle)
Ol' Painless is located in the Republic of Dave, inside Jack's safe. Stealing the key is a good way of getting inside.
Big Guns
Eugene (Minigun)
Eugene is the personal weapon of Brick, one of Reily's Rangers. Either wax the crew, or get it by completing their questline as one of the possible rewards.
"Experimental MIRV" Fatman
The unique weapon of the already rare and limited Fat-Man Nuke launchers, the MIRV is a total gag weapon you have to fire at least once. I don't know exactly what you have to do to obtain it but a friend has it so I've seen it in action. It fires 8 Mini-Nukes simultaneously. Totally unnecessary, but an absolutely riot to unleash.
Energy Weapons
A3-21's Plasma Rifle
It's a pretty straightforward find as it's a quest reward. After discovering Harkness is the modified android, tell him about his true self. Once he awakens to both memories, you can side with him to have him hand over the rifle. It's only benefits are added power and crit chance, but hey, that's quite a benefit. It also comes with maximized durability. (Note: You can still get the "Wired Reflexes" perk in addition to the gun if you do the following. Lie to Harkness and tell him that his secret will be safe with you. He will hand over the gun and the quest will "end". However, Zimmer will still be in the Science Lab. Go tell him that you've found his andoid for the hit in Karma and the awesome "Wired Reflexes" perk. Viola, best of both worlds!)
As you've probably guessed, this weapon counts as a plasma rifle and can be repaired by scrapping spare plasma rifles.
Melee Weapons
The Tenderizer (Sledgehammer)
The Tenderizer is one of the most powerful melee weapons you can encounter early on. It's in the Anchorage Memorial, along with a shitload of god-forsaken Mirelurks. (Oh I do so hate them.) To obtain the weapon, you have to fix the broken door in the sewers. To do so, you'll need access codes which are found behind the two nuka-cola machines that are standing next to each other. It's on the floor directly beneath the workbench. In the same room, you'll find a dead wastelander named Ted. Get the note from his body. Search all the rooms, and in one of them you'll find what looks like a Scrap Metal pile that says "Door Component" instead. Nab it, head to the sewers with all of these in your possession, and you can fix the door for the hammer in addition to a bunch of ammo, a Big Guns skill book, and a Nuka-Cola Quantum.
The Break (Pool Cue)
As the unique of a pretty pathetic weapon to begin with, The Break still isn't very good. But hey, it's a collectible. It's simply sitting on the pool tables in the middle of Paradise Falls.
Highwayman's Friend (Tire Iron)
The unique Tire Iron is found in the Robot Repair Center (basically, the Mechanist's lair) just north of Canterbury Commons. It's found on a one of the shelves in the facility. It's just a more powerful tire iron as far as I'm aware.
Unarmed
Fisto! (Power Fist)
Fisto! is located in the MDPL-16 Power Station, in the Power Substation section. I don't know more about it than that.
Clothing
Armored Vault 101 Suit
The first valuable unique equipment you run into, it's pretty simple to get. When you first enter Craterside Supply in Megaton, make sure you've got a plain-ol' Vault 101 Jumpsuit equipped. You'll get it in the ensuing conversation. If you're wearing anything else (even the various other types of Vault 101 wear), you'll have to buy it from her for about 250 caps, assuming you have low barter. It offers pretty great protection as well as increasing your small guns and energy weapons skills by 2. I am unaware of what items are needed to repair it yourself, unfortunately.
Tunnel Snakes Outfit
Help Butch save his mother during the opening segment of the game and he'll hand you his pimpin' leather jacket. It increases your Melee skill by 5. Wear it to make the Fonz proud.
Sheriff's Duster
Lucas Simms' get-up, the Sheriff of Megaton. It increases your small guns skill by 5 and charisma by 1, just like a plain Regulator Duster. The only way you're going to get it is to ice him. Unfortunately, there's no way to repair the armor yourself. It's not very expensive to fix up, but it's worth noting.
Eulogy Jones' Suit
The personal pimpwear of the pimpmaster himself, you'll have to whack Eulogy Jones if you want to wear his digs. It adds +2 to small guns and +1 to Charisma, but offers minimal DR.
Hats and Accessories
Sheriff's Hat
Do your best Wyatt Earp impression while wearing the hat of Lucas Simms, mayoriff (mayor sheriff!) of Megaton. You'll have to off him to get his cowboy hat. It is a regular hat in that it offers minimal protection while increasing your Perception by 1.
Eulogy Jones' Hat
Want a pimp hat? Here's the gear for you. It's simply on a desk inside Eulogy Jones' Pad in Paradise Falls. It raises Charisma by 1 and offers minimal protection. But you can't put functionality above style, can you?
